Enrollment Instructions

1.  Download the Participation Agreement Letter -click
here to open letter.

2.  Complete and sign Participation Agreement Letter.

3.  Send the completed and signed Participation Agreement Letter to ECM Services via fax to  916 988-2139 or email at
gsmpenrollment@ecms.com.

4.  Payment is to be submitted with your  signed Participation Agreement Letter.  Mail payments to:

                                ORANGEVALE, CA 95662

Once both the signed Participation Agreement Letter and required payment are received by ECM Services  your membership will be approved and you will be receiving a New Member Enrollment Packet.  The New Member Enrollment Packet contains important information about your membership benefits and responsibilities.  Please make it a point to open and review the contents of the New Member Enrolllment Packet as soon as you receive it.
