Enrollment Instructions
1. Download the Participation Agreement Letter -click here to open letter.
2. Complete and sign Participation Agreement Letter.
3. Send the completed and signed Participation Agreement Letter to ECM Services via fax to 916 988-2139 or email at gsmpenrollment@ecms.com.
4. Payment is to be submitted with your signed Participation Agreement Letter. Mail payments to: ECMS PO BOX 622228 ORANGEVALE, CA 95662

Once both the signed Participation Agreement Letter and required payment are received by ECM Services your membership will be approved and you will be receiving a New Member Enrollment Packet. The New Member Enrollment Packet contains important information about your membership benefits and responsibilities. Please make it a point to open and review the contents of the New Member Enrolllment Packet as soon as you receive it.
